Our Financial Policy:

It is important that you completely understand our financial policies.

We are preferred providers for a number of insurance carriers and will bill them directly for you, provided you have signed the proper forms authorizing us to do so. Payment for any deductibles and/or copays is required at the time of service.

Not all insurance plans cover all services. In the event your insurance plan determines a service to be “not covered,” you will be responsible for the complete charge. Payment is due upon receipt of a statement from our office.

Please remember that ultimately your insurance policy is a contract between you and your insurance company and that any claims are filed as a service to you.

If you are insured by a plan that we do not have a prior agreement with, we will be happy to provide you with an insurance claim form so that you may submit your claim. In such case, payment for our services will be required at the time of service.
